Membership
If you would like to be considered for membership of the Association of Specialist Providers to Dentists please click on the attachment below, read the notes and complete the form, returning it with the items requested to the address indicated.
Please note that to be considered for membership it is the individual that will apply rather than the company. You will also need to provide:
a) letters of support from two existing ASPD members, and
b) written testimonials from five individual unassociated dentists that you have worked with.
